Seasons Hospice & Palliative Care is seeking a Patient Family Liaison to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.
The Patient Family Liaison is responsible to sign-on hospice appropriate patients onto the Seasons Programs. This discipline is also responsible information visits and well as following up on pending admissions.
Previous experience in hospice, homecare, healthcare sales and/or Marketing required.
Bachelor's Degree preferred.
Demonstrates excellent verbal and written communication skills.
Demonstrates commitment to hospice philosophy of care.
Demonstrates the ability to work closely with a variety of people effectively using problem solving skills.
Demonstrates a willingness to learn regulations and related reimbursement policies for hospice and long-term care.
